About grant of reading, copy of military career document

 It is officer, civilian employee of the old army and, about military career document of person who had permanent domicile in Kumamoto at the time of the end of the war, issues reading and copying.

 As a general rule, person targeted for grant is former officer, civilian employee. But, in the case of the person death, relative within the third degree can apply.

 Military career document may not be completely kept. When grant of reading and copying is hoped for as it is necessary to confirm yes or no of document, please call by all means beforehand.

 The following filing of paper is necessary for application.

1. When the person (former officer, civilian employee) applies

 (1) Request for military career matter inquiry [style] (can download than ※ following)

 (2) Identification card (copies such as driver's license, health insurance card) of the person (former officer, civilian employee)

 (3) Self-addressed envelope (82-yen stamp pasting) (as issue by ※ mail, please enclose by all means.)

2. When relative (relative in the third degree) applies 

 (1) Request for military career matter inquiry [style] (can download than ※ following)

 (2) Identification card (copies such as driver's license, health insurance card) of applicant

 (3) Family register to prove that old officer and applicant are relatives in the third degree

 (4) Self-addressed envelope (82-yen stamp pasting) (as issue by ※ mail, please enclose by all means.)


   In addition, please inquire for military career document of the old navy relations as follows as Ministry of Health, Labour and Welfare has jurisdiction over.
